A thorough examination is therefore very important. For some causes, targeted therapy is available that can either solve the problem or at least alleviate it. This can halt the development of further heart damage or even restore heart function.<\/p>\n<p>Focus on ejection fraction<\/p>\n<p>Recommendations for the treatment of chronic heart failure are based on a basic classification according to left ventricular ejection fraction. This sounds complicated, but it simply refers to how much blood is pumped out of the total chamber volume during one contraction. A preserved ejection fraction is when the heart pumps out at least half of the chamber&#8217;s volume, a mildly reduced ejection fraction is between 40% and 50%, and a reduced ejection fraction is below 40%. However, even with a preserved ejection fraction, a patient may suffer from heart failure with all the negative consequences that entails. Rather than an ejection disorder, however, the problem is excessive stiffness of the heart, poor filling, and a significant increase in pressure in the filling heart.<\/p>\n<p>&nbsp;<\/p>\n<p>Since these disorders have different manifestations, their treatment also varies.<\/p>\n<p>&nbsp;<\/p>\n<p>Medications for reduced ejection fraction<\/p>\n<p>&nbsp;<\/p>\n<p>Modern treatment for these patients is based on a combination of five groups of medications:<\/p>\n<p>&nbsp;<\/p>\n<p>Beta blockers, which reduce heart rate and the heart&#8217;s energy requirements.<\/p>\n<p>Angiotensin-converting enzyme (ACE) inhibitors. These drugs block the hormonal axis that affects blood pressure and kidney and heart function.<\/p>\n<p>Mineralocorticoid receptor antagonists (MRAs), which prevent sodium and excess water retention in the body by limiting stress signals and counteract unwanted changes in blood vessels and the heart.<\/p>\n<p>Gliflozins are drugs originally intended for the treatment of diabetes, which increase the excretion of sugar in the urine, thereby regulating its blood level. However, it has gradually been discovered that, in addition to diabetics, patients with heart failure and kidney disease also benefit significantly from them.<\/p>\n<p>Diuretics lead to increased excretion of water and certain minerals from the body, thereby removing excess fluid and thus reducing the burden on the heart. They are used in patients with edema. Unlike the first four groups, their use leads to an improvement in symptoms rather than prolonging life.<\/p>\n<p>Medications for mildly reduced or preserved ejection fraction<\/p>\n<p>For these patients, only diuretics have long been recommended to alleviate symptoms and treat associated diseases. However, it has recently been found that these patients also benefit from gliflozins in terms of prolonging life.<\/p>\n<p>Vaccination against influenza, COVID-19, and pneumococcus can save lives<\/p>\n<p>In patients with heart failure, it is essential to prevent so-called decompensation, i.e., an acute, rapid worsening of a chronic condition with a significant increase in symptoms. These episodes often require hospitalization and are an unfavorable indicator of further development despite satisfactory treatment. Respiratory infections are one of the common causes of decompensation. Patients with heart failure are therefore recommended to have regular seasonal vaccinations against influenza and COVID-19, and a conjugate vaccine against pneumococci is also appropriate.
