This complex procedure can significantly prolong and improve quality of life, but it is limited by the availability of suitable donors. In the Czech Republic, heart transplantation has a well-established structure that includes careful patient selection, a demanding surgical process, and thorough follow-up care. An alternative to heart transplantation is mechanical cardiac support, which can also serve as a bridge until a suitable donor is found.

Criteria for inclusion in the transplantation program

Patients with heart failure must meet strict criteria to be placed on the waiting list. The main criteria include:

Terminal heart failure: The disease is at such an advanced stage that other treatments (e.g., drug and device therapy) are not effective.

Age and overall health: Patients should be in a state of health that allows them to undergo major surgery and subsequent immunosuppressive treatment. Active smoking or the use of electronic cigarettes is an obstacle to transplantation.

Mental and social stability: Patients must be able to cooperate with doctors and adhere to the prescribed treatment regimen, which includes long-term medication and regular check-ups.

Heart transplant process

Donor selection and patient preparation: When a suitable donor is found, the patient is immediately hospitalized and prepared for surgery. Compatibility between the donor and recipient is crucial, both in terms of blood type and immunological compatibility.
Surgery: The heart transplant itself is a very demanding surgical procedure that takes several hours. During the operation, the patient’s damaged heart is removed and replaced with a healthy donor heart. This process requires precise teamwork and a high level of expertise.
Postoperative care and monitoring: After the operation, the patient is carefully monitored in the intensive care unit. The following months are very important for the success of the transplant, as the body must accept the new heart. Appropriate immunosuppressive treatment is adjusted, which the patient then takes long-term. It is possible to live for decades with a transplanted heart. Challenges and successes in the Czech Republic

In the Czech Republic, heart transplants are performed primarily at the Institute for Clinical and Experimental Medicine (IKEM) in Prague, and recently the number of procedures at the Brno University Hospital has also been increasing. Both facilities are among the leading centers in this field. Heart transplantation has a long tradition in the Czech Republic and has a high success rate.

One of the main challenges is the shortage of donors, which is a problem not only in the Czech Republic but also worldwide. Raising awareness of organ donation and supporting this noble act are desirable, but in the Czech Republic, presumed consent is enshrined in law – unless a person actively expresses their disagreement in a special register during their lifetime, they are deemed to consent to the possible transplantation of their organs after death. In some foreign countries, the opposite is true.

How to join the transplant program?

If you are a patient with heart failure and are considering a transplant, the first step is to consult your cardiologist. They will assess your overall health and may refer you to a specialized center such as IKEM. A series of tests will then be performed to determine whether you meet the criteria for inclusion on the waiting list. It is always better to look into this option sooner rather than later, as you may not meet the criteria later on due to a deterioration in your overall condition.

Heart transplantation is a treatment option for terminal heart failure when other treatment methods are no longer effective.
